Calf Contouring

Calf Augmentation · Calf Implants

Calf contouring reshapes the lower leg using implants, fat grafting, or a combination, to build volume and improve proportion where the calf muscle appears underdeveloped or asymmetric.

Who this procedure suits

Some people have calves that stay thin and flat no matter how much they train, often due to muscle shape rather than fat or fitness. Calf contouring adds structured volume to the lower leg, improving balance between the calf, knee, and ankle. It does not change bone structure or correct significant leg-length or alignment issues.

A good candidate if

  • Naturally thin or flat calf muscles unresponsive to exercise
  • Visible asymmetry between the two calves
  • A calf that has stayed underdeveloped since childhood or after a past leg injury
  • Realistic goal of improved proportion, not a dramatically different leg shape

Not suitable if

  • Active circulatory or vascular disease in the legs
  • Uncontrolled diabetes affecting wound healing
  • History of deep vein thrombosis or clotting disorders
  • Unrealistic expectations about size or muscle-like definition

How the procedure is performed

Consultation

We examine your calf shape, muscle tone, skin quality, and circulation, and discuss whether implants, fat grafting, or both suit your anatomy.

Preparation

Pre-operative blood tests and a vascular assessment are arranged. You will be asked to stop smoking and certain medications before surgery.

Surgery

Solid silicone implants are placed through small incisions behind the knee, positioned beneath the muscle fascia. Fat grafting, when used, is injected in layers for a smoother contour.

Follow-up

You are reviewed within the first week, then again at monthly intervals as swelling settles and the final shape develops.

Recovery — what actually happens

Day 1-3Rest with legs elevated. Some discomfort and tightness in the calves is expected; pain medication is provided.
Week 1Gentle walking begins for short distances. Compression garments are worn to control swelling.
Week 2-4Gradual return to daily activities and desk work. Avoid running, jumping, or heavy leg exercise.
Month 2-3Swelling continues to resolve. Light gym activity may resume once cleared at review.
Month 6-12Final calf shape and symmetry become apparent as tissues settle fully.

Honest medicine — the risks

Every surgical procedure carries potential risks. Risks vary with the procedure and your health, and the surgeon will discuss them with you along with the measures used to reduce them:

ScarringIncisions are placed in the natural crease behind the knee to keep scars discreet
InfectionSterile technique and antibiotic cover are used around surgery
Delayed wound healingCareful tissue handling and activity restriction protect the incision while it heals
Implant displacement or asymmetryPrecise pocket dissection and correct implant sizing reduce this risk
Nerve irritation or altered sensationDissection stays close to known anatomical planes to protect nerves
ThromboembolismEarly mobilisation, compression stockings, and, when indicated, blood thinners are used
Prolonged swelling or firmnessCompression garments and gradual activity help tissues settle evenly

Frequently asked questions

Is calf augmentation done with implants or fat?

Both options exist. Implants suit patients wanting defined, lasting volume. Fat grafting suits smaller corrections or refining contour, though some fat is naturally reabsorbed over time.

Will I be able to walk normally afterwards?

Yes, though walking is limited in the first week. Most patients return to normal walking within two to three weeks.

Can calf implants be felt under the skin?

Implants are placed under the muscle fascia, which usually gives a natural feel, though this depends on your muscle thickness and skin quality.

Is this surgery suitable for correcting leg shape from birth conditions?

It can help with muscle asymmetry present since childhood or after a past injury, but it does not correct bone alignment or length differences.

When will I see the final result?

Swelling takes several months to fully resolve, so the definitive shape is usually visible around six months after surgery.

Is this procedure right for you?

The only correct answer comes from a clinical examination. Book a consultation — in person, or by video if you are outside the UAE.
